ESIC Application are invited from Registered Medical Practitioner (Modern Medicine with Minimum M.B.B.S. Degree) for empanelment as Insurance Medical Practitioner (IMP) and Private Nursing Homes, Trusts & NGO’s Located in centers of 19 District as per list in Gujarat Providing Primary health Services with basic investigations to the insured person and their families living in areas where new implementation of ESI scheme is under process, age of Applicant for IMP should be less than 67 years at the time of agreement, contract period shall be one year, renewable every year for a maximum period of three years. Fixed Capitation free Rs.300/- per IP family unit per annum will be paid for providing primary heath services with basic investigations.
