Documents to be produced in original at the time of Interview
The Candidate should bring following original documents and one set of self-attested photocopies at the time of Interview with application form:-

1. Identity Proof (PAN Card, Passport, Driving Licence, Voter Card, Aadhar Card etc.)
2. Address Proof (Passport, Driving Licence, Voter Card, Aadhar Card etc.)

iii. Certificate showing Date of Birth. (10th Certificate/ Birth Certificate)

1. Two recent passport size photographs.
2. Class 10th & 12th Certificates.
3. MBBS & Medical M.Sc Mark Sheets & Certificate.

vii. Attempt Certificate
viii. Internship Completion Certificate

1. FMGE Certificate conducted by NBE (For foreign Graduate)
2. Registration with Medical Council of India/ State Medical Council.
3. Experience Certificate (in case of MBBS must have completed compulsory rotatory internship and must produce internship completion certificate)

xii. Reservation category Certificate (OBC*/SC/ST/PH) (*Candidate should belong to non-creamy layer of Central List of OBC).
